- Staphylococcus aureus including MRSA (accounts for ~70%)
- Gram-negative bacilli (infrequent)
- Mixed infections with anaerobes
- Streptococcus spp. including S. pneumoniae
- Mycobacterium tuberculosis
- Immunocompromised hosts (especially advanced AIDS): Aspergillus, mycobacteria (TB, MAI), Salmonella, other Gram-negative bacilli
