Dear Colleague: HIV Surveillance Reports

This Dear Colleague letter announces the publication of two new HIV surveillance reports and an AtlasPlus update. The COVID-19 pandemic in the United States led to disruptions in HIV testing services and access to clinical services throughout 2020. This disruption resulted in a steep, single-year decline in HIV diagnoses that is mostly attributed to declines in testing caused by less frequent visits to health centers, reduced outreach services, and shifting of public health staff to COVID-19 response activities. Given these disruptions, data for 2020 should be interpreted with caution.

The annual surveillance report summarizes information about diagnosed HIV infection in the United States and dependent areas. HIV surveillance data are used by CDC and their public health partners in other federal agencies, health departments, nonprofit organizations, academic institutions, and the public to help focus prevention efforts; plan services; allocate resources; develop policy; detect, monitor, and intervene in HIV clusters; and monitor trends in HIV infection.

This report complements the HIV Surveillance Report and presents the results of focused analyses of National HIV Surveillance System (NHSS) data to measure progress toward achieving national HIV prevention and care goals. Data for persons aged ≥13 years with diagnosed HIV are presented for linkage to care, receipt of care, viral suppression, stage 3 (AIDS) classifications at HIV diagnosis, deaths, and survival. Also included are data on perinatally acquired HIV infection in the U.S. and preexposure prophylaxis (PrEP) data (prescriptions, indications, and coverage) for persons aged ≥16 years.

During 2019-2020, 38% of transgender women surveyed in seven major U.S. cities reported receiving a previous positive HIV test result. Low income (44%), experiencing homelessness (39%), and severe food insecurity (40%) were common and associated with lower likelihood of receipt of HIV prevention and health care; having a health care provider with whom the participant is comfortable was positively associated with receiving those services.

The Centers for Disease Control and Prevention (CDC) and the Health Resources and Services Administration (HRSA) developed the Integrated Guidance for Developing Epidemiologic Profiles: HIV Prevention and Ryan White HIV/AIDS Program Planning to assist with compiling and interpreting HIV prevention, surveillance, and care data for state, territorial, or local HIV epidemiologic profiles. The document provides one set of guidance to help profile writers develop integrated epidemiologic profiles and to advise them how to interpret epidemiologic data in ways that are consistent with, and useful in, meeting the planning and evaluation needs of both HIV prevention and care programs.

This report examines estimates of (1) HIV incidence, (2) prevalence (persons living with diagnosed or undiagnosed HIV infection), and (3) percentage of diagnosed infections among persons living with HIV (percentage of persons living with HIV who are aware of their infection) in the 50 states, the District of Columbia, and Puerto Rico for persons aged greater than or equal to 13 years. Numbers of persons aged greater than or equal to 13 years living with diagnosed infection are reported numbers, not estimates. These numbers are based on diagnosed cases with vital status information reported to CDC through December 2020. Prevalence estimates for the year 2019 are preliminary and based on deaths reported to CDC through December 2020.

The Ending the HIV Epidemic: A Plan for America (EHE) initiative will leverage critical scientific advances in HIV prevention, diagnosis, treatment, and outbreak response. Throughout the initiative, the CDC will routinely release data on the 6 core indicators for EHE to allow for more timely monitoring of progress. The tables included in this report provide preliminary data on HIV diagnoses, linkage to HIV medical care, and preexposure prophylaxis (PrEP) coverage. Data for the 3 indicators are provided at the national-, state-, and county-levels (EHE Phase I jurisdictions only). In addition to being preliminary, data for the year 2020 should be interpreted with caution due to the impact of the COVID-19 pandemic on HIV case surveillance activities in state/local jurisdictions.

The age-adjusted rate per 1,000 PWDH of HIV-related deaths decreased 48% and non-HIV-related deaths decreased 9% during 2010-2017. Differences in HIV-related deaths persist for certain populations.

This report examines census tract-level social determinants of health (SDH) among adults with diagnosed HIV infection in 2018 in the 50 states, the District of Columbia, and Puerto Rico. Data are presented for diagnoses of HIV infection, linkage to care, and viral suppression through 2018 among persons aged 18 years and older by 5 SDH indicators and selected characteristics that include sex, age, race/ethnicity, and transmission category. This report also addresses health disparities along with income inequality and factors for special consideration in addressing health disparities.
