Data are presented among persons with diagnosed, perinatally acquired HIV infection and persons aged ≥13 years with diagnosed HIV for linkage to care, receipt of care, viral suppression; stage 3 (AIDS) at the time of diagnosis of HIV infection; and death and survival of persons with diagnosed HIV infection by selected characteristics that include gender (or assigned sex at birth where indicated), age, race/ethnicity, and transmission category (based on assigned sex at birth). For preexposure prophylaxis (PrEP), the number of persons who have been prescribed PrEP, the estimated number with PrEP indications, and PrEP coverage are presented. In 2021, molecular HIV analysis in Georgia identified clusters of rapid HIV transmission among Hispanic or Latino (Hispanic) gay, bisexual, and other men who have sex with men (MSM) in metropolitan Atlanta. A multicomponent investigation identified factors that might limit access to HIV services, including language barriers, immigration- and deportation-related concerns, and sexuality-related cultural norms. Health departments, providers, and community-based organizations collaborated to address these barriers.

This Dear Colleague letter announces the publication of two new HIV surveillance reports and an AtlasPlus update. The COVID-19 pandemic in the United States led to disruptions in HIV testing services and access to clinical services throughout 2020. This disruption resulted in a steep, single-year decline in HIV diagnoses that is mostly attributed to declines in testing caused by less frequent visits to health centers, reduced outreach services, and shifting of public health staff to COVID-19 response activities. Given these disruptions, data for 2020 should be interpreted with caution.

This report complements the HIV Surveillance Report and presents the results of focused analyses of National HIV Surveillance System (NHSS) data to measure progress toward achieving national HIV prevention and care goals. Data for persons aged ≥13 years with diagnosed HIV are presented for linkage to care, receipt of care, viral suppression, stage 3 (AIDS) classifications at HIV diagnosis, deaths, and survival. Also included are data on perinatally acquired HIV infection in the U.S. and preexposure prophylaxis (PrEP) data (prescriptions, indications, and coverage) for persons aged ≥16 years.

During 2019-2020, 38% of transgender women surveyed in seven major U.S. cities reported receiving a previous positive HIV test result. Low income (44%), experiencing homelessness (39%), and severe food insecurity (40%) were common and associated with lower likelihood of receipt of HIV prevention and health care; having a health care provider with whom the participant is comfortable was positively associated with receiving those services.

The document provides one set of guidance to help profile writers develop integrated epidemiologic profiles and to advise them how to interpret epidemiologic data in ways that are consistent with, and useful in, meeting the planning and evaluation needs of both HIV prevention and care programs.

This report also addresses health disparities and factors for special consideration in addressing health disparities.

This NOFO is a re-announcement of PS22-2202. The purpose of this NOFO is to improve understanding of barriers and gaps in HIV prevention services that may lead to continued HIV transmission or delayed diagnosis of HIV. This NOFO will support enhanced surveillance of people diagnosed with HIV during the earliest and latest stages of infection (i.e., stages 0 and 3, respectively) to understand recent experiences with HIV prevention and testing services, as well as the system- and individual-level barriers that led to these failures in early diagnosis and prevention. Recipients of this NOFO will identify, locate, and recruit all persons diagnosed in the past year with stage 0 or stage 3 infection in their jurisdiction for participation in behavioral interviews conducted by a CDC contractor.
